Purpose

During the lifecycle of a new customer implementation or a new product installation, the project may require the customer to answer a questionnaire before a project task can be scheduled or completed. These forms may ask questions about their company setup, or server setup.

This document discusses the location of these forms and the handling of the completed forms.

Locating the Forms

The docx versions of the forms listed below are all included in this article. They have been set up as fillable Word docs and are locked down from editing except for the fields to fill in. A backup copy is stored in the BG-Project Management Group Teams General channel, File section: Microsoft Forms- New Versions.

Storing Customer Completed Forms

Once the form is returned by the customer, the form should be uploaded to the project File section and attached to any task that requires the information.
