The host license comes from the going to Preferences=> License. Copy that and paste it in there (this will be happening automatically in the future). The URL Prefix is the company, or tenant name. I can be whatever you want, but I would name it after the company, either permar or Midwest. e.g.

Https://permar.sedonacloud.com or https://midwest.sedonacloud.com (if you’re using subdomains)
https://sedonacloud.com/permar or https://sedonacloud.com/midwest
